The Seniors program connects eligible seniors 65 and older with local volunteer ophthalmologists who provide a medical eye exam at no out-of-pocket cost, and up to one year of follow-up care for any condition diagnosed during the initial exam, for the physician services.
The Glaucoma Program provides a glaucoma eye exam at no cost to those who are eligible and uninsured. Those who are eligible and insured are billed normal office procedure, and responsible for any co-payments. (This is an awareness program to provide a baseline glaucoma eye exam to those who may not be aware they are at increased risk).
Short Hills, NJ
New Eyes provides a basic pair of single or lined bifocal lenses. A New Eyes voucher is for individuals who have no other resources with which to obtain a basic pair eyeglasses. Applicants must have an eye exam including their pupillary distance (PD) measurement before applying.

ACC Senior Services (formerly known as The Asian Community Center of Sacramento Valley) is dedicated to enriching the lives of seniors and caregivers.
Housing options: Independent Living, Assisted Living, Skilled Nursing, and Rehabilitation.
Support programs: ACC Rides Transportation Service, Bridge to Healthy Families, Caregiver and Support Services, Friendly Visitor, Meals on Wheels, Resource Center, and Senior Employment Program.
Programs include ACC Nursing and Rehabilitation Center, ACC Senior Services Programs, Meals on Wheels by ACC.
Offers a variety of treatment options that accommodate all levels of need and a full continuum of care. Specialized programs include short and long-term residential treatment, outpatient treatment options, transitional living and case management services with knowledgeable and caring staff who understand the complexities of addiction.
Programs: Promise House (Women’s residential program), Men’s Residential (28-bed residential alcohol and drug treatment facility), Outpatient Program, Sober Living (916-706-2007), Bridges Women and Children (transitional living program exclusively for mothers receiving benefits from CalWorks; 916-410-6112), Specialized Treatment Recovery Services (STARS) (assistance to parents involved w/CPS and Sacramento County Dependency Courts getting assistance meeting mandates given to them regarding drug and alcohol treatment, testing and support).
CCS provides diagnostic and treatment services, medical case management, and physical and occupational therapy services to children and young adults under age 21 with CCS-eligible medical conditions.
Your child may qualify if he/she meets ALL FOUR eligibility criteria: 1) Age eligible: your child is under 21 years old; 2) Medically eligible: your child has a physical disability or medical condition that is covered by CCS; 3) Residentially eligible: your child lives with a parent or guardian who lives in Sacramento County; 4) Financially eligible: your child has Medi-Cal or Healthy Families or the family’s adjusted gross income is less than $40,000 or out-of-pocket medical expenses are expected to be more than 20 percent of the family’s adjusted gross income.
